| The Referral Process ...
Whether
you are a person who feels that DRS could help you personally, or a
professional seeking a service provider of integrity and caring for a
patient or client, the following are basic guidelines outlining our
Referral process.
(a)
FOR THE INDIVIDUAL
Contact
our office to discuss your specific needs and requirements. Our
Office Administrator, Susan Frost, will be pleased to provide any
information and answer any of your questions about our company, fee
structure and services.
(b) FOR
THE PROFESSIONAL
1. Contact our Office Administrator, Susan Frost, for basic
information about our services and/or fee structure. An
Information Package, including our Referral Form, will be faxed or
mailed upon request. You may also download a copy of the Referral
Form from the link below. This form is a two page document
providing us with basic client information, as well as specific
details concerning their medical and psychological
background as appropriate. We also request your complete name,
address, phone and fax number, as well as that of the related
funding agency. The Referral Form must be completed in full
and returned to our office by fax, email or post.
3. In all cases of medical disabilities prospective clients must
have their doctor's permission to return to driving before DRS will
begin any in-car sessions. We wish to let our fellow
professionals know that we will follow their lead and seek to work in a
team relationship on behalf of the client/patient. We are here to
offer a recovery option within our own area of expertise.
4. Upon receipt of the Referral Form, our Office will prepare the OCF 22 -
Application for an Assessment and forward this document to the Patient's
medical practitioner for review and approval.

The signed OCF 22, once received from the medical practitioner, is then processed and faxed to the Insurance Company.
6. When approval has been received from the Insurance Company, the client
will be contacted to arrange an Initial Assessment
appointment. Appointments can usually be arranged within 5 to 7
days. The client should allow 2 to 3 hours for this
assessment.
7. The Initial Assessment:
a)
In-Home Interview: The client will be interviewed to gather
background information about their collision and will be provided
opportunity to express their personal goals for the program as well as
address any issues or questions they may have about the program or
our services.
b) In-Vehicle Assessment: The second half of the assessment
is conducted in-vehicle. The client will be asked to participate
as appropriate to the Referral - as a driver; as a passenger; or
both. The Driving Therapist's vehicle which is equipped with a
dual brake and special visual devices. Consideration is given to the client's physical or
emotional limitations. The proposed program is always discussed
with the client following the assessment.
c) An Initial Assessment Report including Recommendations is prepared and
forwarded to the Insurance Company along with the corresponding OCF 18 - Treatment
Plan (an insurance document that contains details of the proposed
treatment and cost).
8. When approval of the OCF 18 is received from the Insurance Company,
the client will be contacted to arrange a schedule of
appointments to begin their rehabilitation program. The
protocol of our company has always been to travel to the client's home
or workplace, conducting in-vehicle rehabilitation
sessions in a client's own living and working
environment.
The
usual waiting period from the date of a client's assessment to the
commencement of their in-vehicle rehabilitation program is
approximately 3 to 4 weeks. The client receives copies of all
correspondence throughout their program.
